加温湿化吸氧技术对气管插管患者拔管前的气道管理效果

摘要(Abstract):

目的 评价机械通气患者实施T管脱机阶段使用加温器进行热湿化的气道管理效果。方法 将60例使用有创机械通气、经治疗后具备脱机指征的重症患者随机分为T管脱机组(对照组组,30例)和T管加温器脱机组(实验组,30例),对照组使用T管连接可调节输液器,控制湿化液速度,实验组在对照组的基础上,使用加温器置于可调节输液器距T管10cm处,进行加温湿化。观察两组患者痰液粘稠度、刺激性咳嗽、气道出血、痰痂形成情况、是否成功拔出气管插管,SPO2及平均带管时间。结果:实验组在痰液粘稠度,刺激性咳嗽,气道出血,痰痂形成方面,气道管理效果均优于对照组,SPO2实验组高于对照组,平均带管时间实验组低于对照组,P<0.05,差异有统计学意义;两组拔管率无差异,P>0.05。结论:T管加温湿化在机械通气患者试脱机阶段取得了很好的气道管理效果,为拔出气管插管奠定了基础。
Objective To evaluation the airway management effect of mechanical ventilation in patients with the implementation of T test tube offline phase using the heater heat humidification. Methods 60 patients using mechanical ventilation, after treatment with offline indications of severe patients were randomly divided into T tube removal unit ( control group,30cases) and T tube heater dewatering unit ( the experimental group,30 cases), The control group using T tube is connected with the adjustable infusion device, control the humidification fluid velocity, the experimental group in the control group on the basis of, use heater placed adjustable infusion set from T tube 10Cm, the heat and humidity of. Results The experimental group compared with the control group decreased respiratory rate, oxygen saturation than the control group increased, the average time of tube shorter than in other group, P<0.05, subjective comfort, sputum viscosity, irritating cough, airway bleeding, the phlegm scab formation have significant differences, P<0.05; successful extubation rate of two groups, P>0.05 Conclusions T tube warming humidification in mechanically ventilated patients of offline stage obtained good effect of airway management, improving patient comfort, reducing airway irritation, improving the sputum, laid the foundation for extubation
